Bad Bug: Achievement guide/Helmet Nesingwary "The Collected Quests" guide

It hung my display up while it dinged a bell and kept adding this same error over and over.

I see the problems in the guide file and have this on the list to get fixed.

As a work-around you can run the 'The Green Hills of Stranglethorn', 'Hills Like White Elekk', and 'The Snows of Northrend' achievement guides and that will give you the Collected Quests achievement when they are done.
