Achievement/Cataclsym: "From Hell's Heart I Stab at Thee"

Achievement/Cataclsym: "From Hell's Heart I Stab at Thee"
-- 2 errors
(1) It has one of those npc numbers instead of a name.
(2) After I clicked on that achievement quest to begin it,
Zygor took me immediately to Step 2.
[Step 2 said "Congratulations! You have earned the achievement...", etc.]
